Just a comple more questions as we look into the possible issue.
 
What operating system are you running for IPSentry.
To which COM port is the modem assigned (Control Panel / Phone & Modem Options/ Modems)
 
I would like to try going direct to the COM port to bypass a potential driver issue and see if this is related to the TAPI driver of the type of modem.  We have sever USR's installed here, but no Win Internals..
 
To do this, set the paging to COM port - direct to com.
